Literally: A blunder
Idiomatically: A screw up

This is one of those short and sweet idioms which comes directly from the Arabic فَضِيحَة and although slang, is found both in general use and of course in all the newspapers. A Hebrew equivalent would be to say בושות "bushot" or embarrassments.

איזו פדיחה: השר נאם וגילה שהוא בהפגנה הלא נכונה.

"Aizo fadicha: Hasar na'am vegilah shehu behafgana halo nechona."

What a screw up: The minister gave a speech and then discovered he was at the wrong demonstration.

Yeah it actually happened.
